Voice Scribe
Place outreach calls from the patient chart, capture the conversation, and turn it into an attested clinical note that feeds RPM and CCM billing.
Voice Scribe records a patient outreach call, transcribes it, drafts a clinical note from the transcript, and holds that draft until a clinician attests to it. The attested note is what carries your documented minutes into RPM and CCM billing.
Nothing bills until a human signs. The draft is a starting point, not a record, and the platform treats it that way at every step.
Voice Scribe is a plan-gated feature. If the Pending notes item is missing from your sidebar, your tenant is not entitled to it. Contact your Vantrexia account manager.
The workflow end to end
Place the call
Open the patient chart and start an outreach call, or work the queue from Monthly Call Tracker. Vantrexia dials through your connected voice line.
Consent is applied automatically
Before recording starts, the platform checks the patient's state against its recording consent policy. Two-party states get an explicit affirmation prompt. One-party states get a short disclosure. You do not choose this, and you cannot override it.
The call is transcribed
Live transcription runs during the call. Clinical terms are highlighted as they are detected so you can confirm them while the patient is still on the line.
A draft note is generated
When the call ends, the transcript goes to the note generation model and a draft appears in Pending notes.
A clinician attests
The clinician reviews the draft, edits anything wrong, confirms the minutes, and signs. See Reviewing and attesting notes.
Billing picks it up
Attestation writes the documented time to the patient's billing period. The monthly rollup turns that into CCM time logs and billing entries.
Note states
A note moves through a defined set of states. Knowing them makes the worklist readable.

| Purged | An unsigned draft passed the retention horizon. |
Two states are worth calling out.
Skipped exists because "we could not reach the patient" is a real outcome that needs recording. Skipping keeps the patient in next cycle's worklist and touches no billing minutes.
Purged preserves the row while removing the payload. The note ID stays referenced in the audit chain so the record of what happened survives even though the content does not.
What Voice Scribe does not do
It does not bill on its own. It does not attest on your behalf. It does not let you disable the consent disclosure. If a draft is wrong, the correct action is to fix it or discard it, never to sign it and correct later.
