Monitoring
Triage vital alerts, escalate high-acuity findings, and document the time you spend on patient care.
The Monitoring section is where the day-to-day work of an RPM program actually happens. Readings flow in continuously from paired devices; the portal classifies them against the patient's thresholds, raises alerts when something is off, and queues escalations when an alert needs a clinician's eyes. Your job is to triage cleanly, contact patients quickly, and document what you did so the practice can bill for the time.
Alerts
Severity levels, what triggers each one, how to filter the queue, and how to acknowledge cleanly.
Escalations
When to escalate, how to page the on-call provider, and how to document the action.
Daily, weekly, monthly workflows
What a triage nurse does at 8am, what a clinical manager checks on Fridays, and what good looks like.
Documenting for billing
What counts as billable time, how to log a CPT 99457 contact, and when 99458 add-on units apply.
Field labels and button names are written exactly as they appear at app.vantrexia.com. The portal uses the term Completed in place of Resolved in the escalations queue. Both refer to the same final state.
