Patients
Enroll, manage, and offboard patients in your RPM program.
The Patients tab is where your clinical team lives. Everything that happens to a patient (enrollment, threshold tuning, transfer of care, discharge) flows through these screens.
If you're brand new, start with Enrollment. If you're managing an existing panel, jump to Managing.
Enrollment
Demographics, insurance, RPM consent, and the 99453 setup workflow.
Managing patients
Edit demographics, transfer care teams, mark inactive, and archive.
Thresholds
Per-patient vital ranges, with sensible defaults for HTN, CHF, and T2DM cohorts.
Offboarding
Discharge a patient from RPM, retrieve devices, finalize billing, and preserve the audit trail.
All patient PHI in Vantrexia (names, dates of birth, MRNs, addresses, phone numbers, email addresses) is encrypted at rest. Every view of every patient record is recorded in the audit log under Settings -> Compliance -> Access logs.
