What is remote patient monitoring?

Remote patient monitoring (RPM) is a Medicare- and commercial-payer reimbursable program where your patients take vital sign readings at home (blood pressure, weight, blood glucose, pulse oximetry) using cellular-connected devices. Those readings stream into your clinic in near real time. A trained team member reviews them, intervenes when a patient is drifting out of range, and the practice bills for that clinical time each month.

For chronic conditions like hypertension, congestive heart failure, and type 2 diabetes, RPM has become one of the most reliable ways to catch decompensation early, reduce admissions, and improve adherence. For a primary care or cardiology practice, it's also a meaningful and predictable revenue line.

What Vantrexia does

Vantrexia is the staff portal, mobile app, and back office that runs the clinical and billing side of an RPM program for your clinic:

  • Patient enrollment. A guided workflow captures demographics, insurance, RPM consent, and the diagnosis codes that justify each CPT code you'll bill.
  • Connected devices. Cellular cellular devices ship pre-paired. Readings arrive without your patient ever opening an app or pairing Bluetooth.
  • Smart alerts and triage. Out-of-range readings become alerts on the Triage dashboard, sorted by clinical severity. Your team documents the intervention; Vantrexia tracks the time toward monthly billing thresholds.
  • Billing automation. The system tracks setup time toward 99453, device days toward 99454, and clinical management minutes toward 99457 / 99458. Then exports clean claims to your billing partner or directly to the clearinghouse.
  • Compliance. PHI is encrypted at rest, every patient access is logged, and your signed BAA, audit trail, and consent records are retained for the HIPAA six-year window.

How it fits into your day

Most clinics settle into a rhythm that looks like this:

  • Front desk and MAs enroll new patients and pair their devices during a regular visit. Total time at enrollment: about 10 minutes.
  • A monitoring nurse or care manager spends 30–60 minutes a day in Triage reviewing alerts, calling patients who are drifting, and logging the call in the patient's RPM timeline.
  • Providers see escalated cases (typically a handful per week) and approve any plan changes that come out of triage.
  • Billing runs once a month. The Billing tab shows every patient who hit their 16-day device threshold and their 20 / 40 / 60-minute management thresholds, with the right CPT codes already attached.
